New Patient Paperwork:
Our paperwork is very thorough, and allows us to partner with you in restoring hope for your health. The absolute best way for us to do that is to have a detailed understanding of your health journey. Every bit of information about your health, regardless of how small or insignificant, is like another piece of a puzzle. We want to join you in finding any missing pieces so you can putt the entire puzzle together and see the masterpiece you're created to be!
For Chiropractic Visits, use the links below to complete your paperwork. All four forms are required.
For Nutrition Visits, all of our new patient forms are sent through our patient portal, Practice Better, after you submit your Interest Form (linked at the bottom of this page).

Here's what you need to know.
We are located inside the Caddo business complex, on the first floor. When you come in through the main entrance, you will walk straight past reception and take a left in front of the mailroom. Turn right to head towards the restrooms. Walk past the restrooms and then take a left at the hallway. Our door will be ahead on your left.
